 Every Fall athlete needs to see Mrs. Bellerive during clearance dates prior to turning out!—

CLEARANCE DATES:  November 5th, 6th and 7th on the stage during lunches only

PRIOR TO SEEING MRS. BELLERIVE FOR CLEARANCE YOU MUST (if you did not play a fall sport):

1. Fill out athletic eligibility forms online if you did not play a FALL sport. 

(MUST FILL OUT NEW SET OF ONLINE FORMS AFTER 8/1/2019

2. Make sure your physical is current– will not be cleared if physical expires mid-season!

-They are good for 2 years.  Bring in a copy unless Mrs. Bellerive already has one on file from a

previous  sport.

3. Fees:  Athletic: $40 (per sport)   ASB: $30 (per year)  Pay the GVJH Secretaries!

***PAY ONLINE TO AVOID LONG LINES!!! (cut sports can pay after tryouts)

**4.   Basketball and Wrestling:  You need to get a current baseline concussion screening

-Good for 2 years-  Bring in a copy unless Mrs. Bellerive already has one on file from a previous sport.

-ATI will administer screenings on specific dates. Online registration only.  No walk-ins!  Check GVJH
